Team Representative - Recruiter Request/Contact Form - (Recruiters, Scouts, Coaches and Managers) complete the Recruiter Request Form.
Forward the Team Representative (Recruiters, Scouts, Coaches and Managers) request form to [email protected]. Be sure to indicate the name and reference number of the player you are interested in.
Your request will be passed on to the parents of the player who in turn can follow up directly with the Team Representative (Recruiter, Scout, Coach or Manager).
Please Note - Once the form is downloaded (excel document) bottom left of your screen. Simply fill out the Team Representative/Recruiter Request Form and once complete email it to [email protected]
If you can't access excel simply print the form as hard copy fill it out, take a picture of it and send it to [email protected]
